General comments
Bij patiënten met in het afgelopen jaar een infectie of kolonisatie met piperacilline/tazobactam resistente gram negatieve staven: meropenem iv continu 3000 mg / 24h met bolus 1000 mg of 3dd 1000 mg.
Behandelduur: 7 dagen, ook bij HAP / VAP door Pseudomonas aeruginosa. Bij patiënten met CF of immuunsuppressie met onvoldoende klinische respons kan een langere behandelduur nodig zijn.
Bij penicilline allergie: Zie Odin-protocol 'Werkwijze bij antibiotica gerelateerde overgevoeligheid’ (Odin 049799). Bij aanwijzingen voor een ernstige immediate type penicilline allergie: Ceftriaxon iv 2000mg 1dd of continu 2000 mg/ 24h met bolus 2000 mg + tobramycine 1dd 7 mg/kg.
