General comments

Piperacilline/tazobactam: continue infusie of intermitterend toedienen met verlengde infusie (3 uur): Pseudomonas altijd “I”, en dan is verlengde infusie aangewezen.

Tobramycine 1dd 7 mg/kg gedurende 48 uur (zie doseringstabellen). Gebruik bij patiënten met overgewicht (BMI>25) de adjusted body weight (ABW). Het dosisinterval tussen de 1e en 2e  gift is als volgt:

  • GFR > 80 ml/min: 24 uur
  • GFR 50-80 ml/min: 36 uur
  • GFR 30-50 ml/min: 48 uur (eenmalige gift volstaat)
  • GFR < 30 ml/min: contra-indicatie tobramycine
