General comments

Bij pO2 < 70 mm Hg of 8-9 kPa toevoegen prednison:

  • dag 1-5 2 dd 40 mg;
  • dag 6-10 2 dd 20 mg;
  • dag 11-14 1 dd 20 mg.

Bij een milde PCP kan een totale behandelduur van 14 dagen overwogen worden.

Na behandeling secundaire profylaxe tot CD4+cellen >3 maanden >200/mm, zie Profylaxe > HIV-infectie.

Overige overwegingen:

  • Indien mogelijk dient behandeling met cotrimoxazol bij milde hypersensitiviteitsreacties gecontinueerd te worden.
  • Als cotrimoxazol toch gestaakt moet worden kan overwogen worden na vermindering van de overgevoeligheidsreactie cotrimoxazol te herstarten (AII); eventueel middels opbouwschema (BI) of middels dosisreductie (CIII).
  • Cotrimoxazol moet altijd definitief gestaakt worden bij verdenking op het Stevens Johnson Syndroom (SJS) of op Toxische Epidermale Necrolyse (TEN) (AIII).

Dosering cotrimoxazol is gebaseerd op trimethoprim 15-20 mg/kg/dag en sulfamethoxazol 75-100 mg/kg/dag
