Zie doseringstabel onder aan deze pagina. Dosering aanpassen bij klaring <80 ml/min. Denk aan spiegels op dag na starten vancomycine = 2e dag van behandeling. In geval van intermitterend doseren: oplaaddosering 25 mg/kg, vervolgen met 20 mg/kg 2 dd.

Renal function-replacement therapy

CAPD: geen invloed, 15 mg/kg i.v. elke 4-7 dagen; intraperitoneaal diverse schema’s bijv. 2 g ip 1x/5-7 dagen of 1 dd 50 mg via CAPD zak voorafgegaan door een oplaaddosis.

Hemo dialysis: <70 kg: 1000 mg oplaaddosis gevolgd door 500 mg onderhoudsdosis. 70-100 kg: 1250 mg oplaadosis gevolgd door 750 mg onderhoudsdosis. >100 kg: 1500 mg oplaaddosis gevolgd door 1000 mg onderhoudsdosis.

CAV / VVHD: startdosering 15 mg/kg, verder op geleide spiegels

General comments

Therapeutic Drug Monitoring (TDM): 

  • Patiënt wordt ingesteld op AUC (oppervlakte onder de curve) op basis van tussenspiegel (bij continue dosering).
  • Bij continue toediening: afname tussenspiegel op 2e dag van de behandeling. 
  • Bij patiënten met nierfunctiestoornissen dienen spiegels z.s.m. te worden afgenomen.
  • Afnametijdstip altijd noteren op aanvraagformulier!

Streefwaarden spiegels volwassenen:

  • Streefwaarde AUC:  400 - 600 mg*h/L.
  • Spiegel bij continue infusie: 17-25 mg/L.
