General comments
Bij verdenking endocarditis: 3 sets bloedkweken (6 flesjes) afnemen met een interval van 30 minuten.
Vancomycine: dosering op geleide van spiegels, zie Therapeutic Drug Monitoring (TDM) en doseringstabel Vancomycine.
Bij penicilline allergie:
Zie: Allergie voor (beta-lactam) antibiotica.
Bij aanwijzingen voor een ernstige immediate type penicilline allergie:
Natieve klep; subacuut begin: vancomycine iv + ceftriaxon iv 2g 2dd.
Natieve klep; acuut begin: cefazoline iv oplaaddosis 1g, gevolgd door 6g continue per 24 uur.
Kunstklep: cefazoline iv oplaaddosis 1g, gevolgd door 6g continue per 24 uur + vancomycine iv.
